SEATTLE CANINE CLUB
APPLICATION FOR ENROLLMENT

To enroll your dog as a Club member, please download the Application and the Health and Temperament History by clicking on the link below, complete the forms and fax to us at 206.957.9099. Please also ask your vet to fax a current vaccination record. We will contact you promptly to schedule your dog's "temperament evaluation" at your convenience.

Visitors to Seattle Canine Club are always welcome but, for the health and safety of all of our dogs, please do not bring your pet in until we have scheduled our consultation.
